- Mailing address
- Physical address
- Tel number
- Accounts email address
- Name of a contact in accounts
- Purchase Order No. (if required)
Participant details:
- First name of participant
- Last name of participant
- Email address
- Name of course
- Date of course
- City of course
Once we receive this information we will book your students in and send them the confirmations send your school an invoice.
